Stage name — Can be a nickname
Age — Age doesn't have to be the idol's current age
Gender — F or M
Nationality — I.e: Pure Korean 
In school — Yes or No (if Yes what year. All trainees only attend SOPA)
Applying as foreigner — Yes or No (if Yes where from)
Experience — Yes or No (if Yes how many years of training and what company)
Audition song — Name of song
audition dance — Name of song or Random Music
accepted for — Dancing or Singing
while audtion — Anything happened while audition? (Ex. I went into the wrong                       room and saw a Revel actor)
password — Firebird

Nationality — Canadian-Korean
In school — Yes, Junior
Applying as foreigner — Yes, Canada

 After you are accepted you will have an interview at The Red. You will only become an official trainee once you recieve your acceptance letter from Velvet.
